Have you checked your Back Door lately? I have talked at length about the importance of a solid Front Door process that pragmatically handles all unexpected changes that hit your organisation and ensures you are collaboratively agreeing what we can and cant deliver. Sneaking changes in unnoticed and thus consuming much needed capacity. 1 or 2 little ones might be okay, but these always build up and become a major problem.
